This middle aged lady came to an upper endoscopy because of a chronic sideropenic anaemia. The examination was normal, except for this finding in the oral cavity. A gastroscope is surely not the best instrument of examination of the oral cavity - there are other much easier methods for that. However, sometimes there can be some findings of clinical interest also in this area during an endoscopy. What is this finding on the lining of the inside of the cheeks?

Correct answer:

Oral Lichen Ruber Planus also called Lichen Planus

Lichen planus is a skin and mucosal disease the cause of which is unknown. It particularly affects the lower legs but the lesions can be seen on the trunk, glans penis and vaginal mucosa.

The oral mucosa is involved in about 50% of the patients with lichen planus (also called lichen ruber planus). The oral mucosa - seen in these pictures - shows whitish linear lesions, sometimes with erosions.

In the first and third picture a possible cause of this disease is seen, that is dental amalgam restorations, and replacement of these can be an important option in the treatment of oral Lichen planus.
