This quite old man had suffered from severe abdominal pain for many years, ever since he had an accident causing a spinal injury which was thought to be the main etiological factor. This is the finding in an upper endoscopy. What is the lesion seen on the anterior wall of the gastric antrum?

Answer:

    Early gastric cancer. Histologic examination of the surgically removed speciemen showed severe dysplasia, areas with carcinoma in situ and a small intramucosal carcinoma.
