This old lady came to an upper endoscopy as she had suffered from daily heartburn for many many years. The cause of the reflux was a hiatal hernia, an inside view of which is seen in the first image. The second image shows the distal oesophagus 2-3 centimeters and the third image some 5 centimeters above the hiatal hernia. What is this disease in her distal oesophagus - or is it normal?
Answer
Barrett´s Oesophagus
The diagnose is quite easy when one goes further upwards in the oesophagus of this patient. This image shows the finding about 8 centimeters above her hiatal hernia:

Comments:
Looks like a quite NORMAL oesophagus, no Barrett-changes to the naked eye. The first image might
indicate a small protrusion of the ventricular mucosa into the oesophagus, but then again perhaps the picture is taken
right in the middle of the hiatal hernia?
