This lady in her upper middle age had been treated for osteoporosis for years. Despite medication with vitamin D and calcium she was constantly hypocalcemic. During the last year she had suffered from diarrhea and lost 10 pounds of weight. This finding could be a dudenal celiac disease or i am thinking in a duodenal xanthelasm, but my first answer is celiac disease because of the granularity that is seen in the picture.
