This quite old lady came to an upper endoscopy because of indigestion which was probably related to some functional bowel disorders. The endoscopy was normal, except for this finding in the descending part of the duodenum. The patient was followed up and two years later the finding was exactly unchanged. What is this?
Answer:
A sessile premalignant tubular adenoma in the descending part of the duodenum.
I should be removed, of course, but in a very old patient endoscopic or surgical treatment of this lesion might be more dangerous than follow up.
