This quite old man came to a colonoscopy because of intermittent rectal bleeding - due to internal haemorroids. He had suffered from obstipation for many years. In the right colon these mucosal spots could be seen. At first the investigator thought that these might be small aphthous lesions. That was not the case. Whtat is this finding?

Answer

Melanosis coli due to the use of laxatives. The spots are small areas of mucosa for some reason spared from the pigmentation.
